Mobility Impairment Deep Dive Surveys

Birmingham City Council are writing a report (known as a 'Deep Dive') about citizens living with a mobility impairment in Birmingham. This includes information about the prevalence of mobility impairments in Birmingham, associated health conditions and health inequalities, in addition to mapping local services.

You are invited to take part in this questionnaire to collect lived experience data, but we would also be grateful if you are able to cascade the invite to your own networks, etc.

If you choose to participate there are three different groups the team are looking to gain insight from... Professionals, Citizens and Carers. We realise that some may cross over those groups, so please feel free to engage with more than one survey.
